Magazine Subscription Order Form
sbmon.com contains an order form for subscriptions to Small Business Monthly; we require information from you if you are using the order form to subscribe to the magazine. Information asked of you will include contact information (such as name, e-mail, and shipping address), and possibly financial information (such as credit card number and expiration date). If we have trouble processing an order, we use the information to contact you. This information is used for shipping and billing purposes and to fill your order. We will disclose your name, address and transaction information to our fulfillment partner in order to effect the transaction and send you the product you have ordered, and we will disclose your credit card details to the relevant bank or credit card company in order to complete the transaction.

Your e-mail address will be used to notify you of subscription expiration and will contain renewal instructions. They also may be used for SBM surveys conducted by our own research team and offers that we think may interest you. E-mail addresses are not rented to outside firms. We allow you to "opt-out" of receiving e-mail from sbmon.com at the time you subscribe and within each e-mail you receive.

We publish e-newsletters; we never sell or share our subscribers' e-mail addresses.
